  • Accident Hospital Care, such as Surgery and Hospital Admission costs

  • Common Injuries, such as Emergency Dental Work costs
  • Costs incurred as the result of Dislocated and Fractured Bones
  • Costs incurred as the result of Accidental Death & Dismemberment

Yes, the basic plan covers off-the-job accidents for one low rate.  You can choose between the below coverage options:
  • Employee Only
  • Employee Plus Spouse/same-sex domestic partner
  • Employee Plus Child
  • Employee Plus Entire Family
No, although it does have a death benefit to the beneficiary you name, based on the state of your location. The plan pays to help treat and recover from accidents that strike you on or off the job. You also collect if you are seriously injured such as losing a limb.
No—the plan pays in addition to any other insurance you have. This would include emergency room and doctor visits.
